North Idaho is a unique blend of semi-urban communities, small towns, and remote rural areas. Family physicians in this region require broad clinical training and the ability to adapt to a wide variety of practice settings.
The Kootenai Health Family Medicine Residency is committed to preparing graduates who are “rural ready” — equipped with the knowledge, skills, and relationships needed to confidently practice in communities of any size. Being rural ready extends beyond clinical competence and includes understanding rural healthcare systems, community engagement, and the value of long-term relationships.

Highlights of the curriculum:
Our rural curriculum is built on three key components:
- Broad-spectrum training: All residents receive comprehensive training across the full scope of family medicine to prepare for practice in diverse settings.
- Required rural experience: Every resident completes a minimum four-week rural rotation in a North Idaho community. Additional rural elective experiences are encouraged for residents interested in expanding their rural training.
- Annual Rural Education Day: Each year, rural faculty from our partner communities join residency didactics to facilitate rural case simulations followed by a social dinner, providing residents the opportunity to build meaningful relationships with rural physicians and mentors.
Rural Rotation Sites
Residents currently have the opportunity to complete their rural rotation at 10 partner sites throughout Idaho, with new locations added as partnerships develop. Residents are also encouraged to explore additional rural training opportunities that align with their educational goals.
Each site offers a unique clinical experience, including opportunities to:
- Gain extensive emergency medicine and trauma experience in Orofino.
- Work alongside family physicians providing comprehensive maternity care, including cesarean delivery, in Cottonwood.
- Learn about culturally responsive care while serving patients on the Coeur d’Alene Tribal Reservation – Marimn Health.
- Care for patients in mountain communities such as Sandpoint and Kellogg, where outdoor recreation contributes to a broad range of sports and orthopedic injuries.
- Experience the rewards and challenges of practicing full-spectrum family medicine in rural communities with diverse healthcare needs.
Individualized Rural Training
Residents interested in rural practice receive individualized mentorship from our Rural Faculty Advisor, Dr. Crystal Pyrak, who works with each resident to:
- Select rural rotation sites that match their career interests.
- Develop individualized learning objectives.
- Identify additional rural experiences to support their professional goals.
